I understand that the Blood Drawing Skills and Practice Certificate Course/IV Cannulation Skills Certificate Course offered by the Hong Kong Life Development Association requires real-life blood drawing/IV Cannulation practice with other students and agree to abide by the following rules:
Live blood drawing/indwelling intravenous catheterization practice can only be conducted under the guidance of an instructor or with the instructor's approval.
During the real-life blood drawing/indwelling intravenous catheterization process, students must follow the instructor's instructions to ensure their safety.
You cannot refuse to participate in the real-life blood drawing/indwelling intravenous catheter placement practice or be required to participate in the real-life blood drawing/indwelling intravenous catheter placement practice by other trainees.
Students are not allowed to bring their own blood drawing/indwelling intravenous catheter equipment into the classroom for real-life blood drawing/indwelling intravenous catheter practice.
Students are not allowed to take away blood drawing/indwelling intravenous catheter equipment/blood cuffs from the classroom.
＊If students violate any of the above rules, their enrollment will be cancelled and their tuition fees will not be refunded.
I confirm that I clearly understand and am willing to abide by the contents of the above consent form.
Applicants will directly become members of the association and enjoy discounts on individual courses/activities.
All tuition fees paid (except for cancellation of the course) are not refundable under any circumstances.
The personal information submitted by students will only be used to process registration, student affairs and the release of association information. If students wish to change their personal information, please contact the association.
Participants understand and agree that the Association has the right to change the content, location, date and cancel the course.
